This is what a Mormon Temple Recommend looks like. In order to receive a Temple Recommend, you have to pay the Mormon
Church 10% of your income for one year and be a "Worthy Member".
The bottom reads:
When properly signed and endorsed this certifies that the person named is in good standing and may enter the house of the Lord. This recommend expires one year from the last day of the month
issued. Remains the property of the Church. Is not transferrable. Must be surrendered on demand.
